Accessing Your Health Records
At Cribs Hospital, we are committed to protecting your privacy and providing easy access to your health information. In line with India’s privacy and data protection regulations, you have the right to access and correct your personal health records.
You can request your health records by completing a form, either online or at our facility. Once completed, sign and submit it in person, by mail, or via a secure email. Please note that we aim to process requests promptly, with a maximum response time of 30 days.
Requests for proof of birth/death/admission
Birth Certificate
Cribs Hospital does not issue birth certificates. After a birth, we provide a Statement of Live Birth. To apply for a birth certificate, contact your local municipal office or the appropriate government authority.
Death Certificate
Cribs Hospital provides a Medical Certificate of Death, which is sent to the funeral home. For an official Death Certificate, please reach out to the municipal office or registrar in your area.
Correcting Your Health Records
If you notice any errors in your personal health information, you can submit a Request for Correction form to our medical records department. We are committed to ensuring your records are accurate and will respond within 30 days.
